Vendor Authorization Form

Thank you for choosing D&R SnackPit to be part of your event.

To help us confirm your reservation, we must receive written authorization from the event organizer, venue representative, property owner, or other authorized contact permitting us to operate at the designated location.

As conveyed in your tentative email confirmation, this authorization form is due within 5 days of your reservation confirmation date. Reservations that remain incomplete after 5 days will be canceled.
